You may fill it out, save your changes, and email the file to megan@mandmdance.com. *Complete registration requires registration form, signed waiver/parental agreement, and registration fee OR online registration and registration fee.

2023- 2024 Pricing
Fall Session
Annual Registration Fee (Single Student): $25
Annual Registration Fee (Family): $35
Monthly Class Fees (One Class):
**Tuition prices will increase $1 beginning September 1, 2024.
Spring Recital Fee:
**Fees do not include shoes. Hip Hop shoes will be purchased in May/June for a nominal fee.

- $70 per costume (child x small-large)
- $80 per costume (child x-large – adult large)
- $85 per costume (adult XL)

- Two classes - 10% off
- Three classes - 15% off
- Four+ classes - 20% off
- Half year discount: Get 5 percent off when five months of tuition are paid in one installment.
- Full year discount: Get 10 percent off when all 10 months of tuition are paid in one installment.
- Referral discount: Get 25 percent off one month's tuition when a new student registers and gives us your name!
Summer Session
Registration (new students only): $10 per student, $15 per family

Policies
Dress Code
Payment Policy
Registration and tuition are NON-returnable and NON-refundable. Tuition is due the first week of every month--cash and check at the studio and credit card online. A late fee of $5 will be incurred for each week of delinquency. Returned checks will incur a fee of $25. All families must keep a credit card on file. If payment is delinquent by one month, the card will automatically be charged.
Cancellation Policy
Once enrolled, there is no cancellation fee, but written notice of cancellation must be given before the first of the month or you will be charged for the entire month. There is no prorating for cancellations. Costume fees will not be refunded. After spring break, if a class is cancelled, full tuition will be charged through recital (June).
Inclement Weather: In the case of inclement weather, especially during the winter months, follow the Parkway district school closings. If the Parkway School District is closed, M&M Dance Academy will be closed. You will be able to find this information on our website at www.mandmdance.com, on our voicemail, and on Facebook. The first class due to inclement weather will not be rescheduled. Any additional classes will be rescheduled during our spring break (March 18-24).
Absence Policy: In the event of an absence, all class fees are NON-refundable and NON-returnable. Students may make up their absence in another class with a corresponding style and level.
- Hair must be pulled back for all classes.
- Ballet: leotard, pink tights, pink ballet shoes (canvas or leather, full or split sole), bun
- Tap: leotard, tights, booty shorts, jazz pants, tight capris, black tap shoes (kinder and predance wear mary janes; all others wear lace-up or slip on)
- Jazz, Poms, Musical Theatre: leotard, tights, booty shorts, jazz pants, tight capris, slip-on jazz shoes (pee wee and youth levels wear tan; teen levels wear black)
- Lyrical: leotard, tights, booty shorts, modern half ballet shoe
- Hip Hop: clean street shoes, comfortable clothing/active wear (NO jeans)
